He was re-elected for the Bloc Kernes – Successful Kharkiv, the party of then-incumbent Mayor Hennadiy Kernes, in the October 2020 Kharkiv local election, coming in second on the party list. Kernes died of COVID-19 on December 17, 2020, in Berlin.

Kernes’ duties were taken over by City Council Secretary Terekhov after his death. His powers were revoked by the Kharkiv City Council on December 24, 2020. The Ukrainian parliament set the date for the early mayoral election in Kharkiv for October 31, 2021, on March 30, 2021. Terekhov was nominated as the mayoral candidate by the Kernes Bloc — Successful Kharkiv in this election.

Terekhov was proclaimed the election winner with 50.66 percent of the vote by the electoral commission. Mikhail Dobkin came in second place, receiving 28.4 percent of the vote. Observers claimed several irregularities in the election, and nine polling stations’ results were challenged in court. Terekhov was sworn in as the new Mayor of Kharkiv at an extraordinary session of the Kharkiv City Council on November 11, 2021.
