Although he was born in Germany, Addo represented Ghana for seven years starting in 1999. His debut came in a 5-0 victory over Eritrea on February 28, 1999, and he rose to prominence when he captained his country at the 2000 African Cup of Nations. He is one of only two Ghanaians with German ancestry who have played for the African team, the other being Hans Sarpei from VfL Wolfsburg.

On June 17, 2006, at the RheinEnergieStadion in Cologne, Ghana played the Czech Republic in a 2006 FIFA World Cup match, in which Ghana won 2-0. He started for Ghana as a right midfielder in that match. He participated in Ghana’s 2-1 victory over the United States in 2006 at Nuremberg’s Frankenstein.

With his former club Hamburger SV, Addo began his coaching career in 2009. From 2009 to 2015, he worked there as a youth team coach and an assistant manager. After the 2012 and 2013 Africa Cup of Nations and in preparation for the 2014 FIFA World Cup and the 2015 Africa Cup of Nations, Addo was named the head scout of the Ghana national football team, succeeding Ibrahim Tanko.

It was revealed in April 2019 that Addo would serve as a “talent coach” for his former team Borussia Dortmund after serving in a comparable capacity at Borussia Mönchengladbach since 2017. Following the firing of head coach Lucien Favre in December 2020, he was elevated to serve as Edin Terzi’s first team assistant coach. Terzi had previously been elevated to interim head coach.

The 2020–21 DFB–Pokal final saw Dortmund defeat RB Leipzig, giving him his first trophy as a coach.

The Ghana Football Association announced on September 25, 2021, that Addo had been named as one of two assistants to Milovan Rajevac, the new coach of Ghana.

Prior to the team’s 2022 Qatar FIFA World Cup final play-off matches against Nigeria, the Ghana Football Association announced on February 9th that Addo had been named as the team’s interim head coach.

After Ghana and Nigeria drew 1-1 on March 29, 2022, he advanced Ghana (on away goals) to the 2022 FIFA World Cup final in Qatar. On March 25, 2022, the Ghanaian Black Stars and the Super Eagles drew a 0-0 tie.
