Malone, then with Alchem Laboratories, hypothesized that famotidine was targeting an enzyme used by the virus (SARS-CoV-2) to reproduce, and enlisted the expertise of a computational chemist to help design a 3D model of the enzyme based on the viral sequence and parallels to the 2003 SARS virus. Following promising preliminary results, Alchem Laboratories and Northwell Health in New York launched a clinical investigation on famotidine with hydroxychloroquine.

Shortly after the experiment began, Malone departed from Alchem, and Northwell halted the trial due to a lack of hospitalized patients.

Malone was chastised for spreading COVID-19 misinformation, including making unsupported claims about the toxicity of spike proteins produced by some COVID-19 vaccines, using media interviews to popularize ivermectin self-medication, and tweeting a study questioning vaccine safety that was later retracted. He stated that LinkedIn had suspended his account because of posts he claimed questioned the efficacy of various COVID-19 vaccines.
